Maintenance Terms and Conditions
Service must be performed between March 1st, 2018 - December 31st, 2019.
Rebate application must be submitted within 90 days of service and be postmarked by January 7, 2020. Must submit one rebate application form per unit.
Work must be completed by a participating contractor. All installations are subject to an on site quality assurance visit.
Customers are only eligible for one rebate per unit during the program (March 1, 2018 - December 31, 2019)
Applicant must be a Ohio residential customer of Ohio Edison, The Illuminating Company, or Toledo Edison, FirstEnergy’s Ohio utilities.
It is the responsibility of the customer to assure that all requirements for the rebate are met. Failure to provide any of the required information will prevent processing of your application.
Rebate payments are based on the date of service. Customers must abide by the rules and rebate levels in effect on the date of service.
If applicable rebate amount exceeds purchase price, payment issued will be equivalent to the purchase price rather than the stated rebate amount.
The dated sales receipt must match the date of service listed on the HVAC maintenance rebate application form.
Program procedures, requirements and rebate levels are subject to change or cancellation without notice and are subject to available program funds.
FirstEnergy’s Ohio utilities and/or their designees including program administrators and evaluation contractors reserve the right to review installations to verify completion and measure energy savings to ensure compliance with all program requirements. Such reviews will be made at a time convenient to the applicant. Misrepresentation of installation location or measure eligibility may result in forfeiture of the rebate.
Payment will be issued to the account holder and mailing address on record with the utility.
Please allow up to 90 days from the date all required information is received to process your rebate.
Service must be completed in accordance with all laws, codes and other requirements applicable under federal, state and local authority.
You are urged to seek appropriate consultation concerning any tax liabilities that could be associated with the receipt of the rebate.
FirstEnergy’s Ohio utilities and Honeywell International, Inc., their parents, subsidiaries, employees, affiliates and agents assume no responsibility for the performance of the equipment or equipment warranty, the quality of the work, labor and/or materials supplied, and/or the acts or omissions of the participating contractor.
The customer hereby agrees to indemnify, defend and hold harmless FirstEnergy’s Ohio utilities, their parents, subsidiaries, employees, affiliates and agents from any and all liability associated with this project.

By submitting this application, customer agrees to the terms and conditions of this document and certify that the information I have provided is true and accurate. 20. By submitting this FirstEnergy’s Ohio utilities rebate application and providing your telephone number, you agree to be contacted and receive autodialed calls and/or pre-recorded messages at that number (including if that number is a wireless number) on behalf of FirstEnergy regarding your rebate application.
