Purchase date must be between 11/15/2019 and 9/30/2020 to be eligible for a rebate.
Model number selected must match model number on receipt.
If you are unable to find your model number in the dropdown, your unit may not be eligible and you will not receive an instant rebate. Please choose “Not shown” and type the model number of the unit below. Your submission will be reviewed for eligibility.
Please enter the price the customer paid for the unit without $ sign and including any decimal points.
Please only attach .jpg and .pdf files less than 5mb in size.
Please submit a legible receipt in order to receive an instant rebate. If it is not legible, you will be contacted to provide an updated receipt.
